130 Chapter 6 Supplementary Material Supplementary Methods The ‘Age, creatinine and ejection fraction (ACEF) II’ risk score predicts 30-day mortality after elective or emergency cardiac surgery14. The ACEF II score is calculated using the following equation: ACEF II = age/ ejection fraction + 2.0 (if creatinine is ≥ 177 µmol/L) + 3.0 (if emergency surgery) + 0.2 x hematocrit points below 36% If hematocrit is measured in L/L, it should be converted to a percentage (e.g. if hematocrit is 0.40 L/L, it equals a hematocrit percentage of 40%). Higher scores indicate a higher risk of 30-day mortality. The Acute Physiology and Chronic Health Evaluation (APACHE) IV predicted mortality predicts in-hospital mortality for patients on the first day of ICU admission22. Data collection Baseline demographic data, including comorbidities, were collected for all patients. The APACHE IV predicted mortality was calculated for all patients. The duration of IMV, length of stay on the ICU and hospital length of stay and ICU- and in-hospital mortality were collected. In order to correct for possible differences in ICU occupancy rate, we calculated the average daily ICU occupancy rate for each hospital for each year. Then, the occupancy rate on the day of ICU admission for each patient was compared to the reference level of the individual ICU and expressed as a percentage.
