122 Chapter 8 isation can receive compensation for the care provided. Awareness of this flowchart should also be raised among employees who play a role in access to, and financial management of, care. This includes registration desks, Accident & Emergency Departments(A&E), lab- oratories, X-ray and finance departments. Organisation of care A better record of social medical problems can be a good start in establishing an adequate plan for each newly known homeless person. Better International Classification of Primary Care (ICPC) coding and registration can play a part in this. Screening of newly reported homeless people by a street doctor and a nurse can also help. We recommend that not just 14 of the 43 centrumgemeentes - the inter-municipal cooperation between different municipalities - have street doctor consultations, but all of them. Health insurance companies and municipalities would do well to support the social medical consultation hour. 5. Recommendations for future research Recommendations for research on mortality and health problems among homeless people Research on mortality in new cohorts of homeless people is essential to monitor whether health gains can be achieved in one of the most vulnerable groups in the Netherlands. Future evaluations of homelessness action plans should not only look at social effects but should also include health effects, including mortality. This thesis has described that excess mortality is highest among young homeless women. When studying the effects of the Homeless Youth Action Programme 2019-2021, it is important to look specifically at whether there is also a decrease in mortality among this high-risk group. Regular analysis of health problems data, collected during street doctor consultation hours, is important in order to keep track of the main health problems of homeless people and trends over time. Future research on trends in changing substance abuse among homeless people is essential. Such research can be the basis to find out the pros and cons for important issues such as whether or not to legalise substance abuse. A closer analysis of the few hundred homeless people who consistently presented at the street doctor's consultation hours in all three periods in chapter 3 can provide insight into characteristics of the group of homeless people who, despite interventions, do not escape the cycle.
